Research Participation Opportunity

CSU-ERFSA members have been invited to participate in the following research opportunity.

My name is Cindy Cheung, and I am an occupational therapy graduate student at San Jose State University. I am writing to you to request participation from members of your organization to complete a brief survey about the accessibility of community transportation including driving personal vehicles. We are studying the importance of driving and other forms of transportation, and its effects on engagement in daily activities. We are hoping to survey adults, age of 25 and older, who are living in the community.

This survey will inform our research, and fulfill a capstone requirement for our Master of Science degree in Occupational Therapy. More importantly, this survey and its subsequent data will add value to the body of health science evidence, and improve health in our community.

Participation in the survey is completely confidential and voluntary. No personally identifiable information will be associated with the responses to any reports of the data.  No personally identifiable data will be distributed, shared, or sold to anyone and all data will be confidential.

Every person who starts the survey is eligible to enter a drawing for a $25.00 Amazon gift card.

Please let me know if you will be able to help in distributing the survey link. Thank you very much for your time and consideration of our project.

If you have any additional questions or concerns please feel free to contact myself, Cindy Cheung at cindy.cheung@sjsu.edu or my research advisor, Dr. Winifred Schultz-Krohn at Winifred.schultz-krohn@sjsu.edu
